Food item spoiled, adulterated, contaminated or cross-contaminated.

Food not cooled by an approved method whereby the internal product temperature is reduced from 140 degrees Fahrenheit to 70 degrees Fahrenheit or less within 2 hours and from 70 degrees Fahrenheit to 45 degrees Fahrenheit or less within 4 additional hours.
